At Bridging Life you will work with chronically ill patients. Palliative care is available at any stage of a serious illness, not just at the end of life. It works alongside ongoing medical treatments to manage symptoms, reduce stress and improve quality of life for both patients and their loved ones

Responsibilities: 

  • Conduct face-to-face patient encounters in home settings 

  • Perform health status data collection in a comprehensive, systematic manner. 

  • Collaborate closely with the Medical Director and Associate Medical Director on patient visits and care plans. 

  • Collaborate with Hospice and Palliative Care Directors and interdisciplinary team members to ensure operational excellence. 

  • Provide physician backups including prescription writing (e.g., comfort care kits). 

  • Deliver educational initiatives internally and externally, as needed. 

  • Offer palliative care consults, recommend treatment adjustments, and coordinate care with primary care physicians. 

 

Qualifications 

  • Active Nurse Practitioner License (NP) from the Maryland Board of Nursing (MBON) or State Physician Assistant licensure 

  • Basic Life Support (BLS) certification from the American Heart Association 

  • Prior experience in hospice, palliative care, or related medical fields is highly desirable
