Shift: 6:45 AM - 3:15 PM
Experience:
• 24 months of experience with 3 months worked in the last 12 months.
Requirements:
• This role may require floating to additional units and locations
• Local & travel allowed.
• COVID vaccination required after submission. Religious and medical declinations accepted.
• 12 months gap required between for Staff at Program: SimpliFi Program - Advocate Health Midwest and no current placement allowed at Program: SimpliFi Program - Advocate Health Midwest.
• 12 months gap required between for Part-Time at Program: SimpliFi Program - Advocate Health Midwest and no current placement allowed at Program: SimpliFi Program - Advocate Health Midwest.
• 12 months gap required between for Per Diem at Program: SimpliFi Program - Advocate Health Midwest and no current placement allowed at Program: SimpliFi Program - Advocate Health Midwest.

Shift & Scheduling:
• Weekend Rotation: Call every 4th-5th weekend
• On Call, Call Back Requirements: Every 6th weekend, Friday afternoon-Monday morning 1 night a week Response time is 30 minutes
• Will take call at Lakeland Hospital and work at Southern Lakes in addition to Lakeland and potential of working at Burlington Hospital but primarily it is Lakeland and Southern Lakes they will work out of.
• Holiday Requirements: 1 Summer, 1 Winter
Floating Requirements: Will float to two other sites: Southern Lakes & Lakeland; Will get a schedule every week to show where they will float to that week
7 days maximum time off
1 Charge/Lead reference required from within the last 12 months
1 Manager/Supervisor reference required from within the last 12 months
Proof of identification required
Certifications:
• CST (Certified Surgical Technologist (NBSTSA))
• BLS (Basic Life Support)
