Registered Nurse (RN) β Wound Care / Long-Term Care

Schedule: 5:00 a.m. β 1:30 p.m.
Hours: 3, 8-hour shifts per week (24 hours/week)
Weekend Requirement: Every 3rd weekend rotation
Position Summary
Levindale is seeking an experienced Registered Nurse with Long-Term Care (LTC) Wound Care experience to join our clinical team. Under the direction of the Unit Manager, the RN provides direct patient care, assesses and develops individualized care plans, and supports quality patient outcomes through collaboration with interdisciplinary teams.
This role is responsible for delivering age-appropriate nursing care, supervising delegated staff activities, and promoting a safe, patient-centered environment while supporting organizational goals for quality, safety, and clinical excellence.
Key Responsibilities
- Perform comprehensive patient assessments and document clinical findings accurately.
- Develop, implement, and evaluate individualized plans of care.
- Provide specialized wound care treatment and monitoring for long-term care residents.
- Assess and address patient and family educational needs.
- Coordinate discharge planning and transitions of care.
- Perform hourly rounding and maintain patient safety standards.
- Delegate nursing tasks appropriately and ensure follow-up.
- Collaborate with physicians, therapists, case managers, and other healthcare professionals.
- Participate in quality improvement initiatives and peer review activities.
- Maintain current knowledge of evidence-based nursing practices and wound care standards.
- Support initiatives related to Nurse Sensitive Indicators (NSIs) and patient outcomes.
- Adhere to all LifeBridge Health policies, procedures, protocols, and regulatory requirements.
Qualifications
Required
- Current Maryland RN License or eligibility to obtain Maryland licensure
- Basic Life Support (BLS) Certification
- Associate Degree in Nursing (ADN)
- Long-Term Care (LTC) Wound Care experience
- Strong assessment, clinical judgment, and communication skills
Preferred
- Bachelor of Science in Nursing (BSN)
- Previous experience in skilled nursing, rehabilitation, or post-acute care settings
- Wound Care Certification (WCC, WOCN, or similar)
