Job Summary and Qualifications
- Assumes responsibility for the clinical coordination and consultation regarding care across the continuum for all stroke and sepsis patients.
- Consults and partners with nursing leadership as needed to achieve desired patient outcomes
- Assesses educational needs of staff and assists in the development of educational programs, staff education materials, and community education as needed
- Maintains and ensures compliance of continuing stroke education for providers and nursing staff
- Ensures evidence-based screening and treatment protocols are the standard of care within the facility
- Provides appropriate feedback and coaching to staff based on data analysis and observation
- Ensures accurate and timely collection of clinical data, verifying and updating data as needed Shares prepared data with key stakeholders in an understandable and meaningful context to motivate adherence to protocols and continuous improvement
- Co-chairs the Stroke Multidisciplinary Committee and chairs the Sepsis Committee Meetings
- Serves as a resource to staff, physicians, patients, and families
- Completes stroke and sepsis core measure abstraction, ensuring core measure requirements are met and updated for Joint Commission requirements
- Performs a variety of technical and specialized tasks involved with the concurrent and retrospective evaluation of patient care
- Utilizes quality improvement tools, including root cause analysis, to investigate fall outs and generate action plans
- Provides assistance to all hospital departments in implementing and sustaining their quality improvement processes related to stroke and sepsis.
- Assumes responsibility for meeting all regulatory requirements, including Joint Commission Primary Stroke Certification as well as maintaining a state of survey readiness for this program.
- Participates in division-wide collaboration to improve patient outcomes
- Demonstrates working knowledge of AIDET, utilizing with each patient interaction
- Regular, reliable attendance is required to ensure the highest level of patient care
- Follows hospital safety and quality standards and expectations, as it pertains to job performance and patient care
What qualifications you will need:
- Minimum 3 years hospital clinical experience clinical nursing experience; 3 years of recent acute care clinical nursing experience preferred. Proficiency in use of Microsoft Office, proficiency in the use of spreadsheets including pivot tables, graphing and basic statistics; knowledge of Quality Improvement methodology and team facilitation.
- Licensed as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation within 90 days of graduation.
