Experience:
• 24 months of role experience is required with some in the last 12 months.
• Must have at least 24 months of Telemetry Unit experience with 12 months within the past 6 months.
Requirements:
• Candidates must have a Arizona license or compact license (required for submission).
• This role may require floating to additional units and locations
• Travel only, local not allowed. Candidates must live >50 miles from facility to be submitted.
• COVID vaccination required after submission. Religious and medical declinations accepted.
• COVID booster required after submission. Religious, medical, and personal declinations accepted.
• Flu vaccination required after submission. Religious, medical, and personal declinations accepted.
• 12 months between Staff at Program: Medical Solutions Plus Program - (AZ) CommonSpirit Health
• 12 months between Per Diem at Program: Medical Solutions Plus Program - (AZ) CommonSpirit Health

Eligibility as Current/Previous Employee: Cannot have worked at any CommonSpirit location within the last 12 months.
Required Skills/Experience: Must have unit/specialty experience in the past 6 months.
Required Credentials: Travelers are required to pass a dysrhythmia exam through UNA (WITH 2 MAX ATTEMPTS) within 24-48 hours of receiving offer. If they do not pass in 2 attempts, they will be made a DNU for CSH AZ & Sac regions
Any employment gaps greater than 30 days must be explained.
Resume: Each employment history entry must include all the following: detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, teaching facility.
References: At least one reference must be from the same unit/specialty as the job you’re applying to.
Shift & Scheduling: Every 3rd weekend
Floating Requirements: To any tele/medsurg floor.
Facility holidays: 11/27/2025, 12/25/2025, 01/01/2026. Number of holidays allowed off: 1. Start assignment restriction: 14 days
