About the Role:
Provides direct services by means of education, advocacy and services coordination to individuals injured through or exposed to violence in order to extend the psychosocial support services of the Center for Hope Violence Intervention Programs, in collaboration with the MONSE Safe Streets program, PAAVE (Pathways to Advocacy Against Violence Everyday) and other Center for Hope Programs.  POSITION LOCATED AT GRACE MEDICAL CENTER.
 
 
Key Responsibilities:
	- Meets victims and loved ones following violent injury, assesses social and psychosocial barriers and safety and retaliation risks associated with the patient. 
 
	- Maintains information on available community resources, makes appropriate and timely referrals for services and provides ongoing coordination of needed services.
 
	- Maintains all necessary documentation required by hospital, department, and funder.
 
	- In collaboration with other Center for Hope Program staff, provides information and referral to community at large, as well as individual patients.
 
Requirements:
	- 
Education: HS Diploma/GED preferred
 
	- 
Experience: 1-3 years  experience with trauma informed care and advocacy, knowledge in elder abuse, human trafficking and sexual assault, crisis intervention and safety planning skills, familiarity with legal system in terms of restraining orders, elder protection laws, victim compensation and immigration relief options
 
 
Salary requirements $20 to $24/hour based on experience and internal equity for grant funded position
